Every criterion falls into one of three tiers, based on how it's actually treated during underwriting — not how important it feels day to day.

Hard Stop
Blocks the application
Near-universal decline triggers since the 2021 ransomware surge. There's no partial credit here — "MFA for most users" reads the same as "no MFA" to most underwriters. Any open item in this tier drives the overall read to Blocked.
Rate Impact
Moves the price, not the answer
Won't get an application declined outright, but shows up in premium, sublimits, or coinsurance requirements. Open items here drive the read to Conditional even when every hard stop is closed.
Best Practice
Not required everywhere — yet
The direction the market is moving, based on where the most security-mature applicants already are. Doesn't affect the verdict today, but it's usually a preview of next renewal's Rate Impact tier.

Real applications vary by carrier, industry, revenue tier, and prior claims history, and questions change as the market shifts. Coalition, Corvus, At-Bay, Chubb, Travelers, and every other carrier ask their own version of these questions with their own thresholds — this tool reflects the criteria that are consistently documented across that market, not any one of them specifically. Use it to find gaps worth closing before you talk to a broker, not as a substitute for talking to one.
